Health Information Management manages patient records, release-of-information requests, medical-report requests, coding and health-data quality while protecting confidentiality and authorised access.

Three steps to request health information

Identify exactly what you need

Provide the type of record or report you are requesting, the date or period of care, the service involved and enough information for the team to locate the correct record.

Submit identification and authorisation

Patient requests must be made in writing and include the required release-of-information authorisation. Bring valid identification and legal authority if you are requesting information for another person.

Allow the request to be processed

The team will advise how the information will be released. Patient medical-report requests are normally processed within 7 business days; urgent special requests follow a separate review process.

Visit Information

Before your visit

Bring valid identification, details of the hospital visit or admission, and any written authorisation or legal documentation needed for the request.

On arrival

HIM staff will verify your identity, the request and your authority to receive the information before processing or releasing records.

During your visit

Records may need to be located, checked for completeness, reviewed and prepared for authorised release. Original patient records remain under hospital control.

Urgent or safety advice

Health Information staff cannot provide emergency clinical advice. For an urgent medical problem, contact the clinical service or Emergency Room. If an information request is time-sensitive, explain the urgency when submitting it.
